Abstract
Advance care planning (ACP) extends a person’s wishes concerning health care decisions in the event they lose capacity. ACP documents across settings were of low quality compared to best practices and often did not meet legal requirements. GLMM demonstrated relationships between ACP and place of death in a representative sample.
