Community Health Improvement for Equity: Evaluating Inputs and Community System Change Intensity

Population-level health improvement is effected by collaborative efforts to modify and improve community health. Three community-based initiatives utilizing diabetes interventions for vulnerable populations were included for analysis of inputs and associated intensity of community system changes. Results provided evidence of linkages between variables influencing community system changes and projected impact.
