James McKeen Cattell Fund Sabbatical Award Application

Instructions for completion of the Cattell sabbatical award application.

  1. Complete the following application form. We suggest using our headings and spacing the application as needed to complete each item. DO NOT SUBMIT YOUR VITA in addition to or in lieu of any portion of this form.
  2. Recommendation letters: In support of your application we ask that you solicit letters from at least 2 referees. Send them a copy of the memorandum for referees. Please ask your recommenders to please mail their letters to us by December 10. Members of the Cattell Fund Board of Trustees are ineligible to serve as references

  3. Institutional Certification: Please have your department chair or dean complete the institutional certification and mail this form along with your application.
  4. Mail 5 copies of this application and one copy of the institutional certification by December 10 to:Cattell Fund

The deadline for receipt of applications for the 2012-2013 awards is December 10, 2011. If we are missing materials from your application we will send you one reminder e-mail mid-December. It is your responsibility to make sure that we receive your institutional certification and your letters of recommendation.
